If you would like to register for Sleep Scotland's Sleep Counselling Service please download, complete and sign the Registration Form , or contact us and we will send you an information pack by post.


If you are a professional and would like to refer your own client for our service, please ask the family to complete and sign a registration form and post or fax it back to us.
